Overview
During the 89th Texas Legislative Session, lawmakers passed Senate Bill 12, which requires schools to obtain annual parental consent before providing routine physical or mental health services. To ensure your student can continue receiving services, parents/guardians must complete the consent forms in Skyward Family Access.
How to Complete Consent Forms in Skyward
On a Computer
- Log into your Skyward Family Access account.
- Click on Online Forms from the menu on the left.
- Complete the 25-26 Notices of Consent (two forms in total).
- Indicate whether or not you consent.
On the Skyward App
- Log into your Skyward App.
- Select Open in Browser.
- In Skyward Family Access, click on the + sign on the left side to expand the menu.
- Click on Online Forms from the menu.
- Complete the 25-26 Notices of Consent (two forms in total).
- Indicate whether or not you consent.
